Sign the declaration
प्रतिज्ञापत्रावर सही करा
The student and the parent or guardian both sign the Parent's Declaration.
विद्यार्थी व पालक दोघांनी पालकांच्या प्रतिज्ञापत्रावर सही करावी.

Submit and keep the slip
जमा करा व पावती ठेवा
Hand the signed form to the co-ordinator and keep the acknowledgement slip.
सही केलेला अर्ज समन्वयकाकडे द्या आणि पोहोच पावती जपून ठेवा.

What the form asks for
अर्जात काय भरायचे
✓Student: full name, class, medium, mother's name, date of birth, WhatsApp number, email.
विद्यार्थी: पूर्ण नाव, इयत्ता, माध्यम, आईचे नाव, जन्मतारीख, WhatsApp क्रमांक, ईमेल.
✓School: name, address, city, district, state, pin code, contact, email, teacher's name and WhatsApp numbers.
शाळा: नाव, पत्ता, शहर, जिल्हा, राज्य, पिन कोड, संपर्क, ईमेल, शिक्षकाचे नाव व WhatsApp क्रमांक.
✓Parent or guardian: name and address.
पालक: नाव व पत्ता.
